Optimizes and improves cycles for egg donors, oncology patients, women with low reserve, immature oocyte syndrome and empty follicle syndrome

The international medical journal Upsala Journal of Medical Sciences has just published a research paper led by Instituto Bernabeu entitled "Gonadotropin-releasing hormone agonist ovulation trigger — beyond OHSS prevention". It was developed by Instituto Bernabeu gynecologist, expert in ovarian stimulation, Dr. Juan Carlos Castillo with the participation of Thor Haahr, María Martínez-Moya and Peter Humaidan. The goal of this review is to present the advantages of the gonadotropin-releasing hormone agonist (GnRHa) trigger in ovarian stimulation during reproductive medicine treatment. It highlights that its role goes beyond the prevention of ovarian hyperstimulation syndrome (OHSS).

The Manager of the Embryology Unit at Instituto Bernabeu, Dr Jorge Ten, has given a lecture on the Master’s Course in Human Reproduction organised by Complutense University in Madrid (UCM) and the Spanish Fertility Society (SEF). The doctor has been actively involved in course for over a decade. Dr Ten addressed the issue of oocyte vitrification. Instituto Bernabeu is a groundbreaking clinic in this field with over fifteen years of experience of vitrification of both gametes and embryos. Practices such as these need to be performed under a robust vitrification programme and by experienced embryologists. The Master's Course, in which Instituto Bernabeu plays an active role, trains students in highly specialised reproductive medicine skills and updates their scientific knowledge.

A woman’s ovarian reserve is the number of viable oocytes that she has in her ovaries at any given time. This figure conditions the quantity of ova that can be retrieved during in vitro fertilisation treatment. The Embryology and Reproductive Medicine Units at Instituto Bernabeu are leading research aimed at determining if a reasonable number of good quality blastocysts (embryos on days 5 to 6 or development prior to implantation) can be retrieved following culture and observation of oocytes that have not fertilised correctly. In in vitro fertilisation laboratories, around 20% of inseminated oocytes suffer fertilisation failure and 3% of oocytes fertilise abnormally.

Instituto Bernabeu research looks into whether or not ovarian stimulation factors can affect oocyte morphology and membrane breakage types during ICSI

Instituto Bernabeu research aimed to determine if any ovarian stimulation factors can be linked to oocyte morphology and to the type of membrane breakage when intracytoplasmic spermatozoa injection (ICSI) is performed. This assisted reproduction technique is routinely used in in vitro fertilisation (IVF) laboratories. It has the huge advantage of providing a helping hand when male patients suffer from severe male factors and it stops sperm counts from being a limitation. However, the main disadvantage is that the results depend to a large degree on the operator performing the technique. Although specialists are currently able to perform the process securely, there are some variations in the results that they obtain. Differences between oocytes in ICSI are observed even when an egg donation programme is used.

This is the question that a team of researchers from the Embryology Departments at Instituto Bernabeu in Alicante and Albacete and the genetics laboratory at Biotech asked themselves as they proceeded to study the results obtained in almost 400 cycles of in vitro fertilisation (IVF) and intracytoplasmic sperm injection (ICSI). The research work was presented in poster format at the Association for the Study of Reproduction Biology (ASEBIR) Congress, a gathering attended by leading global experts. The research was based on the relationship between the number of oocytes obtained following ovarian puncture and the results of IVF treatment.

Instituto Bernabeu researches the effectiveness of beginning ovarian stimulation protocols in ova donors irrespective of the stage of the menstrual cycle

Instituto Bernabeu has submitted a pilot study involving ova donors to the European Society of Human Reproduction and Embryology (ESHRE). It researches differences in results when conventional ovarian stimulation protocols are applied in comparison with new, no fixed time stimulation protocols that do not take the stage of the menstrual cycle into account. The research entailed an analysis of ova donation cycles which was performed over ten months. It compared stimulation beginning on days 1 to 3 with ovarian stimulation that had no fixed time from day 4 onwards.

Instituto Bernabeu participated in the 75th edition of the American Society for Reproductive Medicine (ASRM) congress held from 12th to 16th October in Philadelphia. It is one of the leading congresses in the world both in terms of the number of participants and in terms of the quality of the work that is presented there. Dr Joaquín Llácer, Medical Co-Director of Instituto Bernabeu, was the clinic representative at the global gathering of experts attended by 9,300 people from 94 countries. It is a not-to-be-missed event in the field.

Pharmacogenetics and Genomics, a specialist journal, recently published Instituto Bernabeu research work into ovarian reserve and pharmacogenetics, a subject in which the clinic is a leading entity and pioneer. The research was managed by Dr Belén Lledó, Scientific Director of Instituto Bernabeu Biotech, and it addressed the relevance of individual screening of the genetic variants in terms of ovarian response prediction. The aim was to determine if the polymorphisms in some genes involved in ovarian follicle maturation can predict ovarian response.

Instituto Bernabeu researches new ways of optimising oocyte retrieval during ovarian stimulation in donation cycles

Instituto Bernabeu has submitted research work about optimisation of ovarian stimulation in oocyte donation cycles to the European Society of Human Reproduction and Embryology (ESHRE). The research involves using oral progesterone instead of the GnRH antagonist, the alternative pharmaceutical drug that has traditionally been used to control when ovulation takes place. The main advantage of this strategy is how easy it is for patients because it entails fewer injections.

The PGT-A test performed by the Instituto Bernabeu genetics laboratory is awarded the highest possible mark by a European evaluator

The genetics laboratory at Instituto Bernabeu has been awarded the highest possible mark in an assessment of its PGT-A genetics test by an external and independent European organism. The assessment was performed anonymously on 56 European laboratories. The PGT-A test assesses if the embryo is chromosomally normal or aneuploidy. It is recommended for women who are older when having children; women who have a background of recurrent pregnancy loss; and women who have suffered from implantation failure. This test is also recommended for couples who have chromosomal abnormalities.

This means that the clinic has been included on the Autonomous Community of Valencia Register of Quality Certifications for Health Clinics, Services and Establishments. It covers, on the one hand, the ISO 9001:2015 certification that the clinic was awarded in January 2016 for ‘Provision of assisted reproduction clinic healthcare, gynaecology consultations and pregnancy control. Performing molecular biology, andrology laboratory, semen bank, oocyte bank and embryo bank activities.’ On the other, the Autonomous Community Register has added UNE-EN 179006:2013 for ‘Management of patient safety risks’, awarded in December 2015.

Instituto Bernabeu researchers have presented experience of a clinical case that describes uterine incarceration, a very infrequent complication during pregnancy. They gave their presentation at the 1st National Congress of the Spanish Society of Gynaecology and Obstetrics (SEISEGO) that was held within the framework of the Spanish Society of Gynaecology and Obstetrics Congress (SEGO). Uterine incarceration occurs during pregnancy when the uterus becomes wedged into the pelvis. There are very few available described cases of this complex and serious condition that is difficult to diagnosis. As such, this study is a very valuable contribution towards improving our understanding of it and advising medical staff about when the symptoms arise.

Research by Instituto Bernabeu investigates if the choice of day for sperm retrieval has an impact on pregnancy results

Azoospermia is one of the causes of male sterility. In order to be able to retrieve spermatozoa in men with this condition, the man has to undergo techniques involving sperm aspiration from the epididymis or testicles, or a testicular biopsy. Although these techniques can be performed at any time and the samples can be cryopreserved for use in the future, with our patients we generally aim to synchronise the procedure with oocyte retrieval. When we do so, the procedure can be performed the day before or the same day as the ovarian puncture that is used to retrieve oocytes from the female partner. Researchers at Instituto Bernabeu carried out a study to find out which of the two days chosen for testicular spermatozoa retrieval gave the best clinical results.  The research work was presented to the 1st National Congress of the Spanish Society of Gynaecology and Obstetrics (SEISEGO) that was held within the framework of the Spanish Society of Gynaecology and Obstetrics Congress (SEGO).

A team of gynaecologists, embryologists, molecular biologists and biologists at Instituto Bernabeu has carried out research to determine if there is an increased number of monozygotic twin pregnancies following transfer when an embryo biopsy during the blastocyst stage (day 5 or 6 of development) has been performed. The twins come from a single ovum or single spermatozoon and they have the same genetic make-up. Pregnancies of this kind are also linked to various perinatal complications.

Instituto Bernabeu has presented research work analysing the clinical results obtained in an oocyte donor programme following random-start ovarian stimulation in comparison with conventional ovarian stimulation to the Sterility and Infertility Section of the Spanish Society of Gynaecology and Obstetrics (SEISEGO). Researchers at the reproductive medicine clinic discovered that there are no significant differences between the results and they highlight that this is an advantage for commencing treatment and synchronising the donor and recipient.

The Rafael Bernabeu Foundation addresses assisted reproduction aimed at avoiding the transmission of genetic disorders at the 4th edition of the National Rare Diseases Congress

The Rafael Bernabeu Welfare Foundation participated in the 4th edition of the National Rare Diseases Congress held in Ibi, a municipality of Alicante, on 30th and 31st October. It addressed the role of assisted reproduction treatment in avoiding transmission of genetic disorders. The congress aims to give people with rare diseases and experts in the field visibility, as well providing a forum for sharing information and analysis.

Dr Andrea Bernabeu participates in a symposium on the role of family medicine in detecting infertility

Dr Andrea Bernabeu, a gynaecologist at Instituto Bernabeu, participated in the ‘Family Medicine: the present, the past and the future’ symposium held on 26th October. It was organised within the framework of the 36th National Congress of Medicine Students (CNEM), 14th international edition, held in Alicante between 24th and 26th October. This collaboration falls within the framework of the Instituto Bernabeu and Miguel Hernández University (UMH) Chair of Reproductive and Community Medicine. One of the main objectives of the Chair is to ensure primary prevention of infertility and to provide general practitioners, gynaecologists and the general population with information on fertility.

Instituto Bernabeu plays an active role in refresher workshops on reproductive medicine held in Albacete

Instituto Bernabeu actively participated in the workshops on reproductive medicine held in Castilla La Mancha, Albacete, on 19th and 20th October. The workshops were sponsored by Ferring. The aim was to share knowledge and latest updates. Lydia Luque, a gynaecologist and head of the clinic in Albacete, presented the event. It involved participation by doctors and biologists from 4 private clinics as well as an additional four specialist hospitals located in the Castilla La Mancha area that have human reproduction departments. “The aim was to review, refresh and share knowledge in order to progress, improve processes and improve management,” summarises Dr Luque.

Andrea Bernabeu, a gynaecologist at Instituto Bernabeu, coordinated the Personalisation and Individualisation of Reproductive Medicine Treatment workshop held in Valencia. Dr Bernabeu was part of a group of experts who participated in the 3rd edition of the Clinical Workshops on Reproductive Medicine. Specialists from different clinics shared their knowledge and updates based on scientific and clinical evidence. The aim was to improve approaches to patients.

The Rafael Bernabeu Foundation presents 15,000 euros in scholarships to six top medicine, nursing and biotechnology students

The President of the Rafael Bernabeu Foundation and Medical Director of Instituto Bernabeu presented six scholarships for a total amount of 15,000 euros to students with excellent academic results and limited financial resources in order to help them continue with their studies. Within the framework of its commitment to society, the Rafael Bernabeu Foundation awarded scholarships worth 2,500 euros each to six medicine, nursing and biotechnology university students in order to cover the cost of fees and study material.

The Instituto Bernabeu is helping out once again this year in the University Master’s on Community Pharmacy Direction and Management & Optimization of Pharmaceutical Care organised by the Universidad Miguel Hernández (UMH). Doctor Andrea Bernabeu has taken part in this fourth edition that deals with aspects related to pregnancy and maternal-child health with theoretical and practical classes. In addition to the classrooms this year, there is a new showroom laboratory that has been fitted out like a real pharmacy so that the students can get some hands-on experience. Instituto Bernabeu has submitted three items of research work to the French Federation of Reproduction Studies (FFER) Congress that will be held in the French city of Lyon between 12th and 14th September. The event will bring together over 600 experts from all over the world who will exchange information on the latest updates in fertility and reproduction.
