La revue scientifique Human Reproduction a publié une recherche de l’Instituto Bernabeu qui étudie si le nombre de copies de l’ADN mitochondrial (ADNmt) dans les embryons humains peut affecter le taux de grossesse. La clinique est la première à réaliser une étude complète de l’ADNmt embryonnaire associé au résultat de la FIV, en analysant la qualité et la quantité de l’ADNmt.

Des recherches de l’Instituto Bernabeu se sont penchée sur l’hypercontractilité utérine avant un transfert embryonnaire et ont étudié si l’administration d’Atosibán était utile chez les patientes qui ont ce problème. Les résultats de cette recherche « Traitement par Atosiban de l’hypercontrcatilité dans le transfert d’embryon » ont été présentés au Ier Congrès National de la Société espagnole de l’Infertilité et la Stérilité (SEISEGO).
Des chercheurs de l’Instituto Bernabeu ont présenté l’expérience d’un cas clinique dans lequel ils décrivent l’incinération utérine, une complication de la grossesse très peu fréquente. Ils l’ont fait au Congrès National de la Société espagnole d’Infertilité et de Stérilité (SEISEGO) qui s’est tenu lors du Congrès de la Société espagnole de Gynécologie et Obstétrique (SEGO). L’incarcération utérine se produit durant la gestation lorsque l’utérus est coincé dans le pelvis et ces cas complexes, qui sont graves et présentent un diagnostic difficile, sont rarement décrits. Cette étude est donc d’une grande importance car elle peut contribuer à approfondir la connaissance et à informer les médecins quant au moment où apparaissent les symptômes de cette complication.
Le directeur médical de l’Instituto Bernabeu, le docteur Rafael Bernabeu, participe au XXème Congrès sur la Propriété Intellectuelle et l’Innovation. C’est une action de promotion au développement de différentes industries à forte intensité d’innovation, qui se déroule au siège de l’Université d’Alicante (UA) situé calle San Fernando 40. La rencontre est organisée par European Intellectual Property Institutes Network (EIPIN) et réunit des experts internationaux dans différents domaines de connaissance dans le domaine de la propriété intellectuelle, l’innovation et les brevets.

La Fondation Rafael Bernabeu ouvre l'appel à des bourses pour étudiants universitaires dotés de 45 000 euros, destinées aux étudiants en médecine, sciences infirmières, biologie et biotechnologie des universités du pays, qui mènent une brillante carrière et dont le revenu familial est un obstacle à la continuation ses études.
Une des causes de la stérilité masculine est l’azoospermie. Dans ces cas, pour l’extraction des spermatozoïdes, il est nécessaire que l’homme se soumette à des techniques d’aspiration de spermatozoïdes de l’épididyme ou du testicule ou bien à une biopsie testiculaire. Bien que ces techniques puissent être réalisées à tout moment et que l’on puisse postérieurement cryoconserver les échantillons pour une utilisation future, pour la majorité de nos patients, nous tentons de les synchroniser avec la récupération ovocytaire. Dans ces cas, l’intervention peut être réalisée le jour précédant ou le jour même de la ponction ovarienne durant laquelle sont extraits les ovocytes de la femme. Des chercheurs de l’Instituto Bernabeu ont développé une étude pour déterminer le moment où les résultats cliniques sont les plus efficaces dans un traitement de reproduction assistée en fonction du moment de l’extraction des spermatozoïdes testiculaires. Le travail a été présenté au 1er Congrès National de la Société espagnole d’Infertilité et de Stérilité (SEISEGO) qui s’est tenu dans le cadre du Congrès de la Société espagnole de Gynécologie et d’Obstétrique (SEGO).
Une équipe de gynécologues, embryologues, biologistes moléculaires et biologistes de l’Instituto Bernabeu a développé une étude pour vérifier si après la réalisation d’une biopsie embryonnaire en phase de blastocyste (5ème ou 6ème jour de développement) il existe une plus grande incidence de gestations gémellaires monozygotes après le transfert. Ces jumeaux proviennent d’un seul ovule ou d’un seul spermatozoïde et partagent la même charge génétique. Et il s’agit de grossesses qui sont associées à diverses complications périnatales.
L’Instituto Bernabeu a présenté au congrès de la Section de Stérilité et de l’Infertilité espagnole (Seisego) un travail dans lequel elle analyse les résultats cliniques obtenus après avoir réalisé une stimulation ovarienne aléatoire comparée à la conventionnelle dans un programme de don d’ovocytes. Les chercheurs de la clinique de médecine de reproduction ont découvert qu’il n’existe pas de différences significatives et soulignent que cela représente un avantage pour commencer le traitement et synchroniser ainsi la donneuse et la réceptrice.

Les aneuploïdies embryonnaires sont une des principales causes de l’échec de la Fécondation in Vitro (FIV). La majorité des erreurs qui conduisent à des embryons avec des altérations chromosomiques sont d’origine maternelle et de nombreuses études signalent l’âge de la mère comme facteur déterminant. Une recherche de l’Instituto Bernabeu présentée au Ier Congrès National de la Société espagnole d’Infertilité et de Stérilité (SEISEGO) est allée plus loin et a étudié si la stimulation ovarienne pouvait avoir un effet sur les altérations chromosomiques de l’embryon.
La revue officielle de la Société espagnole de Gynécologie et d’Obstétrique a publié le travail de fin de Master du biologiste de l’Instituto Bernabeu, Rafael Sellers. Le travail dénommé « Résultats cliniques dans le programme de vitrification sélective des embryons » a été dirigé par le directeur de l’Unité d’Embryologie, le docteur Jorge Ten.

Dans son numéro d’octobre, la prestigieuse revue scientifique Human Fertility reprend une recherche développée intégralement par des scientifiques de l’Instituto Bernabeu qui ouvre la porte à l’accélération du diagnostic des patientes présentant un échec d’Implantation et des fausses-couches à répétition et appuie également le pronostic pour proposer un traitement précoce à ces patientes.
La Fondation Rafael Bernabeu, Action Sociale de l’Instituto Bernabeu, a participé au IVème Congrès National des Maladies Rares qui s’est tenu à Ibi dans la province d’Alicante les 30 et 31 octobre, en abordant le rôle de la reproduction assistée pour éviter des maladies génétiques. Le congrès a comme objectif de donner de la visibilité aux personnes atteintes de maladies rares et aux professionnels qui travaillent dans ce domaine et d’être un espace d’échange d’information et analyse.
La gynécologue de l’Instituto Bernabeu, la docteure Andrea Bernabeu, a participé le 26 octobre au Colloque « La Médicine de Famille : Présent, passé et futur » au sein du XXXVIème Congrès National des Étudiants en Médecine (CNEM) et de la XIVème Édition Internationale qui s’est tenue à Alicante entre le 24 et le 26 octobre. Une collaboration qui a lieu dans le cadre de la Chaire de Médecine Reproductive et Communautaire de l’Instituto Bernabeu et l’Université Miguel Hernández (UMH). Un des objectifs principaux de la chaire est la prévention primaire de l’infertilité et le rapprochement de l’information sur la fertilité aux médecins généralistes, gynécologues généraux et la population en général.
L’Instituto Bernabeu Albacete a participé de manière active aux ateliers en Médecine Reproductive qui se sont tenus en Castilla-La Mancha les 19 et 20 octobre à Albacete avec le sponsor de Ferring. L’objectif était d’échanger les connaissances et de partager les nouveautés. La responsable de la clinique d’Albacete, la gynécologue Lydia Luque, était la personne chargée de présenter ces journées auxquelles ont participé des médecins et biologistes de 4 cliniques privées et de quatre autres hôpitaux spécialisés de Castilla-La Mancha ayant des unités de reproduction humaine. « L’objectif est de réviser, actualiser et partager des connaissances qui aident à progresser, améliorer les processus et également à mieux les gérer », résume la docteur Luque.
La gynécologue de l’Instituto Bernabeu, Andrea Bernabeu, a coordonné à Valence l’atelier de Personnalisation et d’Individualisation des traitements en médicine reproductive. La docteure Bernabeu fait partie du groupe d’experts qui ont participé aux IIIèmes Ateliers Cliniques en Médecine de la Reproduction et dans lesquels les spécialistes de différents centres ont mis en commun leurs connaissances et les nouveautés à travers l’évidence scientifique et clinique. Le tout pour parvenir à une meilleure approche des patients.
Le président de la Fondation Rafael Bernabeu et directeur médical de l’Instituto Bernabeu a remis six bourses de formation pour un montant total de 15 000 à des étudiantes qui présentent un brillant dossier académique et de faibles ressources économiques afin de les aider à poursuivre leurs études. Dans le cadre de son engagement social, la Fondation Rafael Bernabeu a donné une bourse nominale de 2 500 euros à 6 étudiantes universitaires en Médicine, Sciences Infirmières et Biotechnologie pour contribuer au paiement de leurs inscriptions universitaires ainsi que du matériel.
L’Instituto Bernabeu participe au XIVème Congrès des Étudiants de Pharmacie
L’Instituto Bernabeu participe, une année encore au Congrès des Étudiants de Pharmacie (CEFA) qui cette année (2018) célèbre sa douzième édition. Les futurs pharmaciens ont découvert les installations de la maison-mère de l’Instituto Bernabeu à Alicante pour connaître directement l’implication des médicaments dans la reproduction assistée.
La vocation enseignante se trouve dans l’ADN de l’Instituto Bernabeu ; c’est un complément à son travail de soin et d’attention aux patients et de recherche. Ce mois-ci, l’Instituto Bernabeu Albacete participe au Diplôme de l’Expert universitaire en plancher pelvien organisé par l’Université de Castilla-La Mancha. Le cours s’adresse à des spécialistes en médecine physique et rééducation pour les former à travers une vision intégrale des pathologies de plancher pelvien.
L’Instituto Bernabeu et la faculté de Pharmacie de l’Université de Castilla-La Mancha (UCLM) organisent la troisième édition du Master en Infertilité masculine qui se déroule à l’UCLM et à la clinique de médecine reproductive. L’objectif est d’élargir les connaissances en médecine de reproduction centrées sur les problèmes de l’homme pour concevoir ; ces derniers constituant un facteur d’égale importance que l’infertilité féminine.

Une étude développée par l’Instituto Bernabeu a été plus loin afin de découvrir quelle était la meilleure stratégie pour réaliser un cycle de diagnostic génétique préimplantatoire pour aneuploïdies (PGT-A), le transfert à frais ou le différé avec cryoconservation et décongélation postérieure.
Advances in embryo biopsy techniques: IB workshop for biologists
As a part of our continuous training programme, an intensive theoretical and practical seminar entitled ‘Advanced Workshop in Embryo Biopsy: Techniques and tips for usage’ will take place. The workshop is aimed at reproduction biologists who are specialised in micro-handling techniques.
Dans un traitement de reproduction assistée, la stimulation ovarienne conventionnelle se réalise lors des premiers jours du cycle menstruel. Cependant, des études récentes ont démontré qu’il est possible de la commencer indépendamment du jour du cycle menstruel, ce qui est un avantage car cela représente une réduction du temps nécessaire pour débuter le traitement. Ces études apportent un résultat similaire quant à la récupération d’ovocytes en utilisant les deux stratégies.
L’Instituto Bernabeu a développé une recherche dans l’objectif de savoir s’il est possible de prédire le succès d’un cycle de don d’ovocytes en analysant deux paramètres, le nombre d’ovocytes mûrs données qui sont obtenus après une stimulation ovarienne et le taux de ceux qui sont finalement fécondés.
Une étude développée à l’Institut Bernabeu Albacete envisage de tenter la stimulation ovarienne chez des patientes ayant des niveaux d’obtention d’ovocytes plus bas en se basant sur les bons résultats obtenus. Il s’agit d’une stratégie se présentant comme une étape préalable au don d’ovocytes.
Une étude de l'Institut Bernabeu étudie les régions affectant le microbiome vaginal et endométrial
Avec l'incorporation de techniques génétiques, il a été possible d'identifier un plus grand nombre de microorganismes qui ne peuvent pas être analysés avec une culture microbiologique normale et qui composent le microbiome vaginal. Les techniques de séquençage massives constituent un outil puissant pour identifier le microbiome. L’analyse du microbiome est importante car, entre autres, dans le domaine de la médecine de la reproduction, il est possible de détecter s’il existe un micro-organisme susceptible de faciliter ou d’entraver l’implantation de l’embryon.
L’un des paramètres qui permettent de mesurer l’efficacité d’un traitement de reproduction assistée est la morphologie de l’embryon, d’où l’importante de son étude.
Une étude développée dans le laboratoire de biologie moléculaire et génétique de l’Instituto Bernabeu, IB Biotech, présentée au congrès de la Société espagnole de Fertilité (SEF) conclut que chez les jeunes femmes, il existe un risque plus élevé que l’embryon présente des altérations sur des segments chromosomiques.
Une recherche développée par l’Instituto Bernabeu a conclu que l’utilisation de l’héparine, un médicament anticoagulant, améliore le pronostic des cycles de fécondation in vitro (FIV) et ICSI (injection intracytoplasmatique) chez les patientes souffrant de fausses-couches à répétition ou de défauts d’implantation.
L’Instituto Bernabeu a présenté au congrès de la Société espagnole de Fertilité (SEF) une recherche menée par la directrice de l’unité génétique de la clinique, Belén Lledó, qui consistait à l’analyse de l’ADN mitochondrial (ADNmt) dans l’embryon et son lien avec le succès des traitements de Fécondation in Vitro (FIV).
L’Instituto Bernabeu a présenté lors du 32ème Congrès National de la Société espagnole de Fertilité (SEF), qui s’est tenu à Madrid, un travail dont l’objectif a été celui de connaître les médicaments utilisés dans des conditions différentes à celles autorisées dans les traitements de Fécondation in Vitro (FIV) chez des patientes à faible réponse, leur fréquence d’utilisation dans ce cas, et de réviser les aspects légaux de leur administration. L’étude avait pour titre : « Utilisation des médicaments dans des conditions différentes à celles autorisées dans un programme de Fécondation in Vitro : révision des aspects légaux et expérience d’utilisation chez des patientes à faible réponse ovarienne ».

Le comité scientifique de la rencontre nationale la plus importante en matière d’infertilité a accepté un total de 16 travaux de l’Instituto Bernabeu, douze d’entre eux sont les présentations orales des auteurs principaux.

La précision des techniques de séquençage massif améliore la possibilité de grossesse
Un travail scientifique de l’Instituto Bernabeu a enquêté sur les possibilités de grossesse évolutive dans l’implantation d’embryons porteurs de mosaïsme chromosomique. Un aspect controversé car il s’agit d’embryons que possèdent une altération chromosomique sur certaines de leurs cellules et qui, jusqu’à il y a peu de temps n’étaient pas détectées. Grâce à la haute précision actuelle des techniques de séquençage massif ou NGS et array CGH dans le diagnostic preimplantatoire pour détecter les aneuploïdies, sont détectées non seulement les altérations chromosomiques en mosaïque de chromosomes complets, mais également et avec précision des segments chromosomiques.
L’Instituto Bernabeu a été présent cette année encore au VIème Forum méditerranéen, une rencontre nationale d’actualisation pour gynécologues sous le titre de Journée de Reproduction humaine organisée par Merck. La directrice de l’unité de Conseil en Reproduction et Génétique de l’Instituto Bernabeu, la doctoresse Andrea Bernabeu a participé à cette rencontre au cours de laquelle les thèmes les plus actuels en médecine reproductive sont mis sur la table ; sa présentation a porté sur « les nouvelles formes de stimulation ovarienne. » La gynécologue a parlé de nouvelles approches à travers les traitements individualisés, les nouveaux protocoles et les nouvelles formes d’administration des médicaments.

Le 32ème Congrès National de la Société espagnole de Fertilité (SEF), qui s’est déroulé à Madrid, a récompensé le travail du Groupe Instituto Bernabeu « Implication du personnel de soins infirmiers dans les traitements de reproduction assistée, quelle est l’opinion des patients ? », dans la catégorie de communication sous forme de présentation. Le prix est octroyé par le Comité Scientifique et Organisateur du 7ème Congrès National en Soins Infirmiers qui se tient en même que le SEF et la recherche fait partie des 16 travaux scientifiques qu’a présenté l’Instituto Bernabeu au principal congrès national d’infertilité auquel la clinique participe tous les ans avec des études sur les différents domaines de recherche de la clinique.
Le directeur médical du Groupe Instituto Bernabeu, le docteur Rafael Bernabeu, a participé en Italie au Congrès « Progrés en médicine reproductive : Focus 2018 ». L’événement s’est tenu à Palermo, capitale italienne de la science et la vie 2018 et a réuni les spécialistes internationaux les plus pointus en matière de médicine reproductive. Il s’intitulait « la potentialité d’implantation des embryons mosaïques » la technique de séquençage massif de dernière génération (NGS) dont la précision permet de détecter les embryons qui présentent un mosaïsme (qui ont des cellules ayant des anomalies chromosomiques) et d’analyser les possibilités d’implantation pour ces derniers.

L’acte de clôture de la troisième édition et d’inauguration officielle de la quatrième a eu lieu à l’auditorium ADDA d’Alicante, coïncidant ainsi avec le VIIIème Congrès National de Pharmaciens Communautaires, organisé par la Société Espagnole de Pharmacie Familiale et Communautaire (SEFAC).

Instituto Bernabeu applies new technique to detect alterations in sperm production
At the Association for the Study of Reproduction Biology (ASEBIR)'s 9th Congress held in Madrid, Instituto Bernabeu introduced a research paper focusing on checking the MLPA (Multiplex Ligation-dependent Probe Amplification) technique's sensitivity levels in order to find out how specific alterations can affect sperm production. Our investigation entitled: "New technique to detect Y chromosome microdeletion (YCM): Clinical implications” was based on the fact that Y chromosomes comprise different genetic regions involved with spermatogenesis or sperm production. In the event that one of these regions, also known as AZF, was deleted or duplicated, this could result in either a low amount or the complete absence of sperm.
The aim of the study led by Instituto Bernabeu gynaecologist, Francisco Sellers, and endorsed by the Association for the Study of Reproduction Biology (ASEBIR), was to determine if there are differences between the quantity of trophoblastic vascularisation during early stages of spontaneous pregnancies and pregnancies after assisted reproduction techniques (ART).

Advances in embryo biopsy techniques: IB workshop for biologists
As a part of our continuous training programme, an intensive theoretical and practical seminar entitled ‘Advanced Workshop in Embryo Biopsy: Techniques and tips for usage’ will take place next September. The workshop is aimed at reproduction biologists who are specialised in micro-handling techniques.
The researchers look at the selection that can be made of chromosomally normal embryos with preimplantation genetic screening (PGS) by using the next-generation sequencing (NGS) technique. This leads to an improvement in the results of in-vitro fertilization (IVF) since it detects chromosomally normal embryos, those which are abnormal and also those known as mosaic, which have both normal and abnormal cells. In the case of mosaic embryos, studies show that the abnormal cells are replaced with normal cells and that they may be implanted and are able to develop into a healthy child, although the success rate is somewhat lower.

Instituto Bernabeu decreases the number of injections required for ovarian stimulation
Instituto Bernabeu has revolutionised the way in which drugs for ovarian stimulation are used. Three days' worth of medicine can be taken in a single injection thanks to a device known as the i-Port which is already being used by diabetics. During courses of fertility treatment and when ovarian stimulation lasting an average of ten days is carried out, drugs need to be administered using injections. Many of the women who come to our clinic admit to being afraid of needles.

The international scientific committee at the European Society of Human Reproduction and Embryology has selected the research that will be made public during the 33rd edition of the annual congress to be held between 2nd and 5th July in Geneva.
Patients undergoing treatment in the egg donation programme and patients undergoing frozen embryo transfer need to be administered progesterone. It is necessary for implantation and for the posterior progress of the pregnancy. Drugs can be administered either by means of a daily subcutaneous injection or vaginally over a period of at least three months. Given that this is a significant period of time, Instituto Bernabeu believes that it is key that the female patient feels at ease if she is to have a good quality of life.

Dr Joaquín Llácer, Director of our specific Unit for the treatment of Poor Ovarian Reserve participated in the Second Congress of Reproductive Health Group (RHG) held on the 7th and 8th October at Daresbury Park, UK.
- In less than two decades, the average age at which couples have their first child has been pushed back from 26 to 31 years, coinciding with the onset of ovarian ageing and increased difficulties in getting pregnant.
- On the first day of the congress, experts from all over the world analysed the issues from a genetics point of view in order to understand why ovaries age, biomarkers for personalising treatment, progress in selecting the best embryo and also the associated emotional concerns.
- The second day covered recent progress in the embryo implantation process and various studies aimed at prolonging a woman's fertility.

Implantation failure is one of reproductive medicine's biggest challenges and it is for this reason that our unit which specialises in its treatment is working on and researching new approaches. One of the new lines of research include approaches in order to reduce uterine contractuality so that embryo acceptance is facilitated during transfer.

IB Research: A new adenomyosis ultrasound marker
Adenomyosis is a condition that is difficult to diagnose and it is far more common than it is often thought to be. Diagnosis needs to be based on clinical data and confirmed using imaging techniques.
The challenge faced by our unit which specialises in treating implantation failure and recurrent pregnancy loss is to search for therapeutic alternatives and to progress in our understanding of correct embryo implantation. The point at which the embryo is transferred is the climax and there is still much to learn about it from a scientific point of view.
We have always known that embryos, as living things, are very sensitive to changes in temperature. Therefore, throughout the entire process in the in vitro fertilisation laboratory, we have systems that keep the temperature stable, except when actual embryo transfer takes place and, at which point, the embryo is taken from the incubator for transfer to the uterus.
Instituto Bernabeu's commitment to giving patients the best chance of achieving pregnancy means we necessarily need to personalise each treatment and involve each department from each phase of assisted reproduction treatment in exploring new options, however insignificant they may seem. This can, sometimes, mean the difference between success and a negative beta result.

The 32nd ESHRE Annual Meeting, the most significant congress for assisted reproduction specialists, will be held in Helsinki, Finland, between 3rd and 6th July, 2016. An embryo biopsy can potentially damage the embryo and it is for this reason that, at Instituto Bernabeu, we study different ways of optimising the options of achieving pregnancy amongst our patients. In this particular case, we studied how the position of inner cellular mass (ICM) during an embryo blastocyst biopsy affects clinical results.
